Admissions Nurse

Schedule: Evening Shift | 3:00 PM – 11:00 PM (Monday – Friday)

An esteemed, purpose-driven senior living and healthcare organization in Connecticut is in search of a committed, meticulous Admissions Nurse (RN) to supervise and streamline clinical intake for new residents.

Specializing in evening admissions (3:00 PM – 11:00 PM), this position collaborates directly with hospital discharge planners, attending physicians, facility management, and families to ensure smooth and safe transitions into the center. By concentrating on incoming clinical assessments, medication reconciliation, and initial electronic chart setups, you empower floor staff to provide direct resident care while setting a standard of clinical excellence from the outset.

Key Responsibilities:

• Lead clinical intake and onboarding procedures for all evening admissions (3:00 PM – 11:00 PM)
• Perform initial nursing assessments, skin evaluations, and thorough admission documentation upon resident arrival
• Verify hospital discharge summaries, attending physician orders, and pharmacy profiles for immediate medication continuity
• Collaborate closely with hospital discharge planners, interdisciplinary care teams, and resident families to prepare rooms and specialized clinical equipment
• Initiate preliminary personalized care plans and communicate critical resident needs to nursing supervisors on duty
• Orient new residents and families to facility policies, care team structures, and clinical routines to ease transition anxiety

Qualifications:

• Licensure: Active, unrestricted Registered Nurse (RN) in the State of Connecticut
• Clinical Experience: Minimum of 1 to 2 years of nursing experience in a skilled nursing facility (SNF), long-term care (LTC), or sub-acute rehabilitation environment preferred
• Admissions Capability: Previous experience in admissions, hospital discharge coordination, or unit management/desk nursing highly desired
• EMR Proficiency: Hands-on experience with Electronic Medical Record systems (e.g., PointClickCare) for accurate, rapid chart initiation
• Communication: Outstanding organizational, clinical prioritization, and interpersonal skills to establish quick trust with families and hospital representatives

Note on Compensation: Final compensation will be determined during the interview process based on license type, depth of clinical experience, and leadership background.
