Nurse Practitioner

Compensation: $131,000 – $170,000 annually (based on experience)**Relocation assistance available**

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available)

Location: Greater Sacramento area, CA (Multiple locations available)

We are looking for a dedicated Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to offer comprehensive primary care services in a community health center. This role allows you to work in a collaborative environment with physicians and multidisciplinary care teams while serving diverse and underserved populations.

Key Responsibilities

• Conduct thorough patient evaluations, including history and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and therapies within your scope of practice
• Develop and implement treatment plans in partnership with the Medical Director
• Monitor patient progress and adjust care plans as needed
• Initiate emergency procedures when necessary
• Offer preventive care, health education, and counseling services
• Maintain accurate documentation following agency and regulatory standards
• Collaborate with healthcare professionals to deliver high-quality, coordinated care

Qualifications

• Active California RN and Nurse Practitioner license
• Current Nurse Practitioner board certification
• Current DEA certification
• Current BLS certification
• Preferred 1–2 years of clinical experience, especially in community health or underserved communities
• A strong dedication to serving low-income, homeless, or medically underserved populations
• Comfortable managing three patients per hour
• Experience with EPIC and in-basket management
• Fluency in English and Russian, Farsi, Pashto/Pushto, or Spanish is a plus
