Key Responsibilities:

  • Conducting comprehensive psychosocial assessments, mental health screenings, and diagnostic evaluations for patients and families.
  • Providing evidence-based crisis intervention, short-term counseling, and medical social work services to support patients facing acute or chronic medical conditions.
  • Collaborating with physicians, mid-level providers, and nursing staff to ensure seamless care transitions between different healthcare settings.
  • Supporting, counseling, and educating patients and families on disease pathology, coping mechanisms, and healthcare decisions.
  • Identifying barriers to health and safety, conducting emergency interventions when needed, and arranging post-discharge care.
  • Maintaining accurate documentation of patient encounters within the electronic health record system.
  • Participating in multidisciplinary team rounds to discuss cases and promote holistic patient wellness.

Qualifications:

  • Master of Social Work (MSW) degree from an accredited institution.
  • Current, active license as a Licensed Clinical Social Worker (LCSW) or eligibility for immediate licensure in Oregon.
  • 1–2 years of clinical social work experience, ideally in a hospital or healthcare setting.
  • Comfort and interest in working in a rural community.
  • Strong crisis intervention, communication, and collaborative skills.
